Terms of Agreement and Privacy Policy

No information you share with Open Hands DPC or Dr Keller will be intentionally shared with any other party unless you give express permission to do so. However, recognize that email, text, phone and video conference are considered unsecured means of communication. Any information you provide via these interfaces is your responsibility. To maximize your care and communication, a release to use such forms of communication will be provided. If you choose to complete a form through the website prior to signing the release agreement to use unsecured means of communication, your completion of the form will be accepted as permission for Open Hands DPC to view any information you provide regardless of completed release agreement or not.

Payments will be made through PayPal or Venmo prior to treatment. Please visit www.paypal.com or www.venmo.com to view their security protocols.

Video calls will be facilitated through Google Meet. Please visit www.support.google.com to view their security protocols.

Prior to treatment, regardless of membership status, you will be asked to complete documentation via DocuSign to setup the direct agreement between you and Open Hands DPC in order to receive services. If you choose, your pertinent historical information can be completed through this interface.

We take your care and privacy seriously. You may, however, notice that a HIPAA form is not included in your documentation to sign/complete. Rest assured your information will not be shared without your express permission first. Yet, since Open Hands DPC does not electronically submit anything to insurance or third party payers, we are not labeled a “covered entity” under HIPAA law.
