Joining is simple.
Text “I’m interested in joining Open Hands DPC” to 859-359-2142.
We don’t have a fancy automated system, so we will text you back as soon as possible (usually within just a few minutes…unless we’re helping someone else).
We will need your FULL NAME, DATE of BIRTH and EMAIL address to send you our New Patient Agreement. Once we have that, we will get you set up for payment and scheduled to be seen.
When completing the paperwork, you will choose your desired membership tier (learn more here).
