DOT Physicals

    1. The physical form with pages 1 & 2 COMPLETED. (Print Form)

    2. Drivers License

    3. Your necessary medical records (see list below).

    1. Upon arrival provide your payment for your physical, any necessary medical records and the physical form with pages 1 & 2 completed to Dr Kelli.

    2. You will then step into the bathroom to give a urine sample and change into appropriate clothing for the exam.

    3. After that, Dr Kelli will review your history and medications with you followed by the exam.

    4. Once the exam is completed, you will be given the original copy of the physical form. An electronic copy will be kept on file for at least 3 years with Open Hands DPC.

    1. A list of ALL MEDICATIONS you take (including over the counter) should be included on your form.

    2. If you have DIABETES - Bring your most recent A1c level and at 3 months of glucose monitoring.

    3. If you have HEART DISEASE - Bring your most recent echo (heart ultrasound) and any records concerning your heart in the past year.

    4. If you have a HISTORY of SMOKING or LUNG DISEASE - Bring your most recent pulmonary function tests and notes from your lung doctor.

    5. If you have SLEEP APNEA - Bring your CPAP records showing use over the past 6 months or more.

    6. Other records may be needed once your history is reviewed or exam is completed by Dr Kelli.

    1. If your exam reveals a more in depth eye exam is needed - Form MCSA-5871.

    2. If applying for a hearing exemption: FMCSA Process Letter.

    3. If applying for a seizure exemption: FMCSA Process Letter.

$100 per exam
