Insurance Breakdown*

Your first visit with us will require we get to know you and set clear expectations for privacy, care, treatment and payment options. Once this initial paperwork is completed, you can expect the following based upon your current coverage agreement(s):

 

Commercial Insurance

Although all services provided in and through Open Hands DPC are covered under the direct contract between you and the DPC, other services (labs, imaging, referrals to other physicians and medication needs) can be processed through your insurance as they typically would under any other model of care.

Self Pay

The DPC model provides a reliable and affordable option for you and your loved ones to receive medical care. We will gladly work with you to find affordable options for labs, imaging, referrals and medications when necessary.

 

Medical Cost Sharing

The DPC model provides a reliable and affordable option for you and your loved ones to receive medical care. We will gladly work with you to find the most affordable options for labs, imaging, referrals and medications when necessary. The costs incurred can then be reported to your medical cost sharing company and evaluated based upon the services you have opted into.

Medicare

In order to serve Medicare recipients and offer low cost medical care to everyone, Dr Keller has opted out of participating as a Medicare Physician. Beginning Oct. 1, 2020, after signing a direct contract acknowledging that neither our office nor you personally will submit any services performed in or through Open Hands DPC to Medicare for payment, your other imaging, labs, referral and medication needs can still be submitted to Medicare. CMS has set the date for such agreements to take place as October 1, 2020 - sorry for any inconvenience this may cause.

 
 

Individual

Medicaid

Unfortunately, at this time it is illegal in Kentucky for recipients of Medicaid to seek out a direct contract with any physician or other provider - even if cheaper, direct pay options of medical care exist . In order to receive care through Open Hands DPC, Medicaid recipients will need to explore other options of coverage.
